Workers’ comp document checklist

Keep copies of your claim form, letters, medical work-status notes and important dates together. A simple folder and a short timeline can make your next conversation easier.

A general guide for California workers. For help with your own situation, contact DWC or find legal help.

Documents to keep

Check off the records you have, or print this list. Your selections are not saved or sent to Nometi.

File names

Include the date and document type in each file name, such as “2026-10-20 — claims administrator letter.” Keep the original copy too.

Dates to record

Write the date, what happened, and where to find the supporting record. For example: “October 20 — received a letter — letters folder.” If you are unsure of a date, say so in your notes. Separate the date printed on a letter from the day you received it.

Before a phone call

Have your claim number, the relevant letter and your questions nearby. Afterward, note who you spoke with, the date and what you understood the next step to be. Ask for clarification if an answer is unclear.

Storing documents securely

Use a secure folder or account you control. Avoid posting claim numbers, medical records or identity documents in public forums. If someone asks for a document, check who they are and how to send it securely.

This list is for organizing your paperwork. It is not an official state form or a list of evidence required for your claim.
